Instructions for Uploading SBIRT Intervention Group GPRA Data
Use the template below to upload GPRA data for all patients who screened positive, received a SBIRT intervention (Brief Intervention [BI], Brief Treatment [BT], or Referral to Treatment [RT]), and have a documented AUDIT, DAST, or CRAFFT assessment score.
Report the highest level of SBIRT service the patient received.
